The en bloc technique was first developed as a method to take out ruptured silicone implants without infecting the body with silicone gel. This procedure involves taking out the capsule (the scar tissue that forms and surrounds the implant) and breast implant combined as one with the aim that no residual tissue is left behind.

The en bloc capsulectomy typically begins with an inframammary incision pattern, which is formed horizontally beneath the natural fold of the breast. If the patient previously has an inframammary pattern from her original breast augmentation surgery, the surgeon will try to make use of the same incision site to minimize new scarring. The surgery usually takes less than two hours and is performed with the patient under general anesthesia.

Once both the breast implant and capsule have been taken out successfully, the body starts a natural process of detoxification. Additionally, drains are placed to stop the accumulation of fluid and check the growth of new capsular tissue. The drains are helpful in restoring the breast to its original shape.

At times, breast lift is undertaken along with breast implant removal to eliminate extra loose skin. Breast lift reduces the lax skin, and repositions and reforms the breast tissue to an aesthetically pleasing position on the wall of the chest for a more youthful appearance.
